What Is It?
D-Mannose is a member of the sugar family, so many people may find it odd that it has the power it does in terms of treating urinary tract infection (UTI). It is mildly sweet in taste and occurs naturally in certain fruits, such as cranberries, peaches and apples. D-Mannose has, based on some research, performed equally as well as or better than antibiotics in treating UTI.

Why we use it?
UTI's or Bladder infections, are a common problem , especially among women. They are the most common complication of pregnancy and are also common in men with prostate enlargement, as these conditions can prevent the bladder from emptying completely. When urine is left stagnate in the bladder there is a higher risk of infection. Urine is normally quite sterile, but it is possible for bacteria, viruses, and fungi to enter the urethra and travel up into the bladder.

Over 90% of UTI's are caused by a specific bacteria called E-coli,which is naturally present in other areas of the body, such as the digestive tract and colon, but does not belong in the urinary tract. Our body will try to flush the E-coli out with urine, but these clever bacteria have evolved, and have developed a binding mechanism, to adhere to residues (of mannose) on the wall of the bladder and save themselves from being flushed away. This is where D-Mannose comes in...

How it works
D-Mannose when present in the urinary tract, acts as a decoy. Bacteria bind to it, instead of the bladder epithelium and are then much more easily removed from the urinary tract, both treating and preventing (in cases of recurring infection) UTI's from developing or spreading to the kidney, which can be a much more serious problem.

How do I use it?
D-Mannose is available in both tablet and powder options. The powder can be mixed with water (to help increase fluids, which is also recommended during UTI). Whether in powder or capsule form, 1-2 g every 3-4 hours is the typical recommended dosage for acute treatment of UTI. A lesser dosage may be recommended for prevention. It is important to remember that although it is the most common cause, not all UTI's are caused by D-Mannose and therefore there are some UTI's that D-Mannose will not be effective in treating. D-Mannose has an excellent safety profile and can be used during both pregnancy and breastfeeding. It will not effect blood sugar in individuals with diabetes.

Cranberry

Cranberry has been well studied for its ability to prevent and decrease the severity of urinary tract infections. It contains compounds called lectins that prevent bacteria from sticking to the walls of the urinary tract. The bacteria are then flushed out through the urine and do not lead to infection. Cranberry also acidifies the urine, which makes it an uncomfortable environment for bacteria to live and reproduce.
Cranberries also contain significant amounts of vitamin C, which is important for the healing of the lining of the urinary tract. Finally, the proanthocyanidins that are found in cranberry have been shown to have anti-cell growth effects in laboratory testing. More studies are needed to determine if cranberry will be clinically useful for patients.

Cranberry can be purchased as a raw berries, encapsulated dried berries or concentrated juice. The concentrated unsweetened juice is the best way to consume cranberry for its medicinal qualities. The juice is very tart but be sure to dilute the juice with water, not other fruit juices. Likewise, avoid drinking sweetened cranberry cocktails because they often contain little actual cranberry juice and lots of sugar.

Sugar actually feeds the bacteria that cause urinary tract infections. Encapsulated cranberry ranges in strength and quality. Purchase a product that gives an equivalent value of raw cranberries or concentrated cranberry juice. Combination products for urinary tract infections may contain cranberry.

Probiotics

Probiotics, or friendly bacteria, are the beneficial microbial flora we all need for optimal digestive and colon health. We all have them the first probiotics being given to us during the process of breast feeding from our mothers, with our need for them never waning throughout our entire lives. Without health gut flora, poor assimilation of nutrients, intestinal irritation or constipation can result. But, their effects go beyond those commonly associated with digestion.

Both naturopathic and medical doctors are recognizing the importance gut flora plays for overall health. Many normal, bodily functions require healthy gut flora, ranging from immune function to heart health. Certain strains of the Lactobacillus type have been shown to be crucial for producing important immune factors which the body cannot produce without them, while others have been shown to be crucial for the reduction of bad cholesterol.

In general, probiotics can be categorized by where they are used by the body. For example, there are friendly bacteria which are used only in the small intestine and those that proliferate only in the large intestine, or colon. But, believe it or not, beneficial bacteria exist in the mouth, urinary tract, stomach, and even on the surface of the skin.

What to Look For in a Probiotic
There are literally thousands of probiotic supplements available in health food stores today; with so many available it can be difficult to know which one is best for you or your family. A multi-strain probiotic is frequently the best way to go for most people. This type of probiotic can be considered the "multivitamin" of the probiotic category of supplements as they will usually supply you with at least 6, 8 or even as many as 10 or 20 different strains of healthy bacteria. Naturally in your body you have many, so it makes sense to take a supplement with as broad a spectrum as possible as this will give you the greatest coverage to replenish or maintain the natural gut flora.

That said, there are certain types of therapeutic probiotics which more specifically address certain health concerns. Saccharomyces boulardii is one that is particularly useful for those who have had chronic intestinal flushing from c. difficile infection or even from food poisoning. In years past we would have ingested these from root vegetables grown in the ground, but in our modern sanitized world where we purchase cleaned vegetables from supermarkets, it's difficult for us to get it from food unless we're growing them ourselves.
