UNDERSTANDING PSORIASIS & NATURAL TREATMENTS

Q. I have psoriasis on my legs and elbows. What do you suggest for a natural treatment? I scratch sometimes so much I bleed. Can you advise?

A. Psoriasis is a chronic condition, that often comes and goes (flares) over a lifetime. It generally is first diagnosed between 15-25 years of age, although can affect an individuals of all ages. Psoriasis, like most skin conditions, can be stubborn to treat, frustrating those who are suffering without relief. Immuno-suppressant drugs are often prescribed for acute action, but are not suggested and can be harmful as a long-term treatment option. The good news is there are a variety of natural treatment and prevention suggestions that can reduce inflammation and irritation. No more scratching.

What is Psoriasis?
Psoriasis is a fairly common immune mediated skin condition that causes redness, scaling and often severe irritation. Skin cells, which grow deep within the many layers of skin, normally rise to the surface about once every 4 weeks. During which time we are constantly shedding the old skin cells that are currently lying on top. In individuals with psoriasis, this process happens too quickly, anywhere from 2-10 times as fast as it normally should. Skin cells then build up excessively on the epidermis, or top layer of the skin, causing the white or sometimes referred to as silvery scales/plaques.

Although there is a genetic component to the development of psoriasis, many consider it an autoimmune condition. One theory is that T cells become active (it is unclear why, although this process could occur with an overactive or dysfunctional immune response) as they would in infection. These T cells move to the dermis of the skin and trigger the release of cytokines (like TNF-α), which cause inflammation and the rapid production of skin cells.

Common Triggers
There are a variety of triggers that may lead to a flare-up or exacerbation of an active process. These include injury to the skin (a breaking of the skin), leaving skin more susceptible to entry by bacteria or virus. Another trigger is stress. Stress reduction and effective coping techniques can go a long way in helping to prevent future outbreaks and manage acute attacks. Dry air and dry skin are also triggers. Consider using a humidifier in your bedroom at night, especially during the winter months. Alcohol and even certain foods can also be triggers, more on this to come. Lastly, too much or too little sunlight can both trigger a flare-up. Although it is crucial not to burn, individuals with psoriasis must make it a priority to get outside uncovered and enjoy the sun for about 15 minutes each day during the warmer months.

Treatment options

There are natural products you can also use, both internally and for external application, that can soothe and help to clear up psoriasis flare-ups. Internally, regularly use a good fish oil, combined with either evening primrose or borage oil, and flax oil. Most combination products do not contain enough DHA and EPA, so it is best to supplement with a fish oil separately. You can alternate the GLA from borage or evening primrose oil with the ALA in flax oil. One day, use flax oil on its own or in salad dressing, etc., the next day aim to get about 1000 mg of GLA. Continue to rotate between these two. Take the fish oil daily, aim to get anywhere from 3-9 g of EPA +DHA/day (not total omega 3 – read the label!) These essential fatty acids not only act as effective anti-inflammatories, but there is some research to suggest that individuals suffering with psoriasis are prone to EFA deficiency. Supplementation can help to decrease redness and irritation, regulate the inflammatory response and serve to prevent future flare-up.

High dose vitamin D has also been shown to be effective in treating psoriasis, although it appears that topical vitamin D analogue creams may be as, if not more effective than internal vitamin D intake. These creams, Dovonex being a popular pharmaceutical brand, are thought to slow the rate at which skin cells multiply. If you are looking for a more natural product consider avocado oil, a natural source of vitamin A and D, or some of the psoriasis specific topical treatments mentioned below that contain natural vitamin D. Another supplement to consider is a B-complex, as this can help to support the stress response helping to potentially prevent a flare-up before it even starts.

There is also some research to suggest that individuals with psoriasis may also be low or deficient in digestive enzymes and/or HCL. Talk to your naturopath about testing your HCL and whether supplementation with digestive enzymes, including HCL might be suggested for you. If nutritional deficiencies exist due to poor digestion, supplementing may help to improve these nutrient levels.

Topically, there are natural creams and ointments formulated specifically for psoriasis relief. Look for a cream that contains soothing ingredients such as calendula, chamomile and aloe vera. Psoriaderm, by DermaMed includes these and extracts of fumaria. Research devoted to the use of fumaric acid esters for the treatment to psoriasis are showing promising results.

Dietary and Lifestyle Suggestions
In terms of diet, food allergies must be considered when dealing with any autoimmune OR skin pathology. Try to systematically eliminate and reintroduce some of the more common allergenic foods, such as wheat, dairy, corn and eggs. Citrus fruits may also aggravate psoriasis in some individuals. Try to avoid refined sugars and processed foods whenever possible.

Sea salt water baths, using warm (not hot) water, can not only be relaxing and stress reducing, but can also serve to soften thick scaly skin. Alternate with oat baths. Put a cup of oats in a cheesecloth (or you can use pantyhose) and put it right in the bath. Squeeze the oats from time to time while bathing. The oats will also help to soothe and soften inflamed skin. Exercise (particularly working up a sweat) can also help to regulate stress and soften skin. Psoriasis, unlike other stubborn skin conditions, like fungal infection is a dry condition as opposed to a condition that thrives in wet/damp environments. The condition itself then leads to even more dryness. This makes moisturizing so vitally important. Whether it be with a specialized psoriasis product, avocado oil or similar, make sure it is not drying, it is all natural and it contains no perfumes are harsh chemicals that can serve to further irritate the skin. Also watch for hairsprays (esp. if the problem occurs on the scalp), face creams and body washes, as well. Try to use all-natural products when and wherever possible.
